Please read :

• Please open your form in a pdf application otherwise you won’t be able to upload your documents.
• All your file uploads should be in ‘jpeg’ or ‘jpg’ format.
• Once you complete the form with all your crew members added, please upload the pdf below and click submit.
• If you have any change in your crew then you can edit your form and submit it again.
• Please ensure that your form is no larger than 20-25 MB; ideally it should be between 10-15 MB so it doesn’t take too long to upload.
